If order is sent by fax, please include:
Credit card number, expiration date, credit card name and billing address, or institutional PO number, plus order information (Cat no, description, number of units), plus shipping address.
Please also include email contact address so you can receive timely order confirmation and FEDEX tracking information. A contact telephone number is also needed for FEDEX shipping.
If you are an academic institution and are not set up for credit card ordering, you may submit a PO and receive an invoice with the shipment (please provide billing address).
